Key Responsibilities
Lead Strategic Population Health Initiatives - Direct development and implementation of data-driven health management programs
Cross-functional Leadership - Facilitate collaboration between departments, vendors, and community partners to achieve population health objectives
Analytics and Outcome Management - Conduct population assessments and gap analyses to identify health trends and priorities
Program Design and Evaluation - Develop and implement targeted health interventions using quality improvement methodologies
Stakeholder Management - Serve as primary liaison for regulatory requirements and community partnerships
Performance Monitoring - Track program metrics and maintain compliance documentation
Requirements
Education
Bachelor's degree in health care or related field required; Master's degree (MPH, MHA) preferred
Experience
Two (2) years' experience in project/program management or Population Health Management initiatives
